Your medical record is a confidential document. It is
the policy of this practice to maintain security of
personal health information at all times and to ensure
that this information is only available to authorised
members of staff. |

Sterilisation |
|
All
instruments used in this clinic are autoclave
sterilized. This clinic operates with the Australian
and New Zealand standards (4815) in sterilization
disinfection and infection control and uses the
Meditrax system to record autoclave procedures. |

Billing Information |
|
The
Swansea Rd Clinic bulk bills all Repatriation &
all full pension & old age
pension card holders as well as patients who are
80 yrs and over.
All other patients are charged
the surgery fee including other Pension Card holders
& Health Care Card holders who will be charged a
discounted surgery fee. The standard Private fee is
$55. An accounting fee of $10 will be charged if the
account is taken without payment on the day.
On weekends & public holidays the $70 fee will be applied for those not
qualified to be bulk billed. Please note that with there is a larger medicare rebate for these days.
Payment is expected at the time of consultation and
can be made by eftpos, credit card or cash. The only
cheques accepted are Bank or Medicare cheques or
postal orders made payable to attending doctor.
Mediclaim online is available to all full paying
clients who have registered their bank account
details at a Medicare office.
Fee for a Referral letter without an appointment will cost $15 payable at the time of collection.
Transfer of Medical Files Fee
Fee will take into account Registered mail, photocopying & collation costs & will need to be paid by the patient before the files will be transferred $10 to $35 depending on size & time taken to collate.
